Saturday 9:00 - Noon (East, West, and Lawrenceburg locations.)
Sunday 9:00 - Noon (West location only. East and Lawrenceburg offices are closed on Sunday)
All offices are closed on major holidays.

New Patients

We are now accepting new patients. We accept most major insurances. Please call the office if you have any questions or to schedule an appointment.
If you are coming in for your first office visit, you will need to arrive 15 minutes before your appointment time to fill out new patient registration forms. For your convenience, these are provided here to complete and bring with you. Registration forms
Please print these forms, fill them out, and bring them with you to your appointment.

Appointments:

Physicals and well-child visits should be scheduled in advance to ensure that you will be seen by your preferred provider and to make sure all the appropriate blood work is done.
Sick visits are scheduled the same day whenever possible. We advise you to call early for an appointment. Walk-in patients are seen on a first-come, first-serve basis AFTER scheduled appointments and emergencies.
Please make an appointment for each person that will be seen. We need to make sure an adequate amount of time is scheduled for each patient to be seen. Please do not ask the provider to see additional patients that do not have an appointment.
Please arrive on time for your appointment and if you need to cancel, please call the office as soon as possible. Frequent no-shows for appointments may result in discharge from the practice.
We ask that you bring any forms that you may need the provider to complete in addition to the following:
1) Your insurance card.
1) Photo Id.

Emergencies

If you have an emergency, please go to the nearest emergency room or call 911.
If you have an urgent problem after hours, please call the office and you will be given the on-call provider's number.

Phone messages and Refills

When calling and leaving a message, please allow an adequate amount of time for your provider to return your call.
We require 24 hours notice to refill prescriptions. Please call at least two days before running out of your medication in case there is a problem with your refill.

Payment for services

We accept cash, check, or credit cards (Visa, MasterCard, American Express, and Discover).
Self-pay patients are required to make a deposit of $56.00 prior to being seen. After your visit you will be billed for the remainder of the charge.
Patients with insurance requiring a co-pay are to pay at time of service and have their insurance card available upon request.
